What You’ll Need to Replace a Lost or Stolen Title
Required Documents:
- Valid government-issued ID
- Last 5 digits of the Vehicle Identification Number (VIN)
- License plate number
- Completed REG 227 – Application for Duplicate Title (we’ll help with this)
- Lienholder release (if vehicle is financed and lien has been paid off)
- Notarization (if required for out-of-state or lienholder involvement)
